PATIENT NOTICE

Our Treatment Room is moving to an appointments system from November 2014. Please contact the surgery in advance to make an appointment with the Nurse. Many thanks for your co-operation.

Prescriptions

PLEASE NOTE; We require 48 hours notice for all repeat prescriptions. Chemists are now charging a fee for emergency supplies of medications.

Emergencies
Emergencies during surgery hours telephone 8772 2621. Between 8.30 and 6.00 pm, urgent requests will be given prompt attention by the receptionist and the doctor will be informed.

Urgent Appointments
If you have an urgent problem, although you will be able to see a doctor the same day, it will be the first doctor available and may not be your usual doctor.

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone the surgery.

PRIVATE MEDICAL SERVICES 

non_nhs_services1Medical examinations for life assurance, pre-employment medical, HGV licenses etc and completion of private medical forms are undertaken by arrangement with the doctor.  These and certain other services are not covered by the National Health Service and will attract a fee.  Please contact reception for an appointment and advice regarding the fee. 

The practice does NOT routinely countersign passport applications or provide sickness certification for Institutes of Further Education, but will with written consent from the student provide information on direct contact from the Institute. 

As the GP is contacted directly by the DHSSPS for medical evidence in relation to benefit claims the practice does NOT complete benefit forms unless it specifically states a doctor must complete the form.
